    Description

    We are currently seeking a Registered Nurse (RN) in the Denton area who is passionate about working in the home health setting on a full-time basis.

    The Home Health Registered Nurse (RN) will be responsible for providing nursing care to patients in their homes, collaborating with the interdisciplinary team, patients, families, and referring agencies, as well as coordinating the overall care delivery.

    • Conduct initial and ongoing comprehensive assessments and treatments focused on the patient's physical, functional, psychosocial, and environmental needs.
    • Evaluate criteria for admission and re-certification in home health care to determine the appropriate levels of care and patient eligibility.
    • Develop, implement, and update the care plan to guarantee continuity and quality of care, making necessary revisions when needed.
    • Offer guidance and educate patients and their families on self-care practices, nutritional requirements, and dietary considerations.
    • Assess the caregiver's capacity to address the patient's immediate needs throughout the care process.
    • Initiate preventive and rehabilitative nursing procedures as required.
    • Adhere to infection control protocols and standard precautions to prevent disease transmission during care activities.
    • Provide clear instructions to Home Health Aides and LPN/LVNs to ensure consistent and high-quality service delivery.
    • Document progress notes reflecting advancements towards established objectives.
    • Communicate changes in patient needs and intervention outcomes to physicians and other team members while evaluating patient and family responses to care.
    • Monitor assigned cases to ensure compliance with third-party payer regulations.
    • Facilitate effective communication among patients, families, colleagues, and other healthcare professionals.

    Requirements:

    • Current RN License
    • Valid Driver's License
    • CPR Certification
    • Valid Auto Insurance
    • Minimum of 2 years of experience as a Home Health RN
    • Preferably familiar with HomeCare HomeBase
