Job description:
We are seeking an LVN Case Manager for a nursing facility. The Case Manager completes weekly status updates, quarterly reports, and care planning with the treatment team.
Job Qualifications:
- LVN
- Supervisory experience recommended
Responsibilities:
- Reviewing admissions
- Weekly updates of facility physician
- Assisting Patient Care Plans on admission, quarterly, and significant changes in status.
- Monthly updates of Patient Care Plans.
- JOC Calls
- Coordinating weekly rounds meetings.
- Weekly updates of team lists.
- Regular chart audits to ensure monthly progress notes are up to date.
- Assisting the nursing team in dealing with patient needs.
- Any other duties as assigned by the Director of Nursing
COVID-19 considerations:
- Initial screening interview conducted online. COVID19 precautions and training upon hire. COVID19 vaccination, most recent booster and weekly testing currently required by LA County Public Health and CDPH.
