Description

Community Resource Advocate/Floor Assistant will provide case management services to a case load of 40-50 residents at Inwood House, a 150-unit, independent living apartment community for persons with a disability and seniors.

Our goal is for our residents to have live the lives they choose, while living in an inviting and supportive community.

Responsibilities:

Assist and engage with individual residents in obtaining and maintaining the services and benefits needed to maintain or increase their independence and ability to age in place.
Resident assessment, service planning, information, referral, coordination of services and advocacy Empower residents to meet their own needs and manage their health and services through participation in service identification, delivery, and management.
Implement new services or increase availability of existing services to meet the needs of residents by developing connections with service providers and a service directory.
Address social and educational needs of the community on advocacy, services, benefits, health, and wellness; and assist family members and the community in understanding the changing needs of residents as they age.
Complete initial introduction, assessment, resident orientation, and other move in activities within established management guidelines.
Maintain an ongoing relationship with residents through proactive outreach, formal, or informal interactions to reach all residents on your caseload monthly. Connect with residents after a hospitalization or major health change.
Resident's information is to be kept confidential following privacy guidelines.
Perform any other duties that may be assigned to you by your supervisor or Manager.
Assist with planning and conducting festive events for the residents.
