Description
Floral Home Care located in Queens county is looking for experienced Intake Coordinator.

The Intake Coordinator is responsible for for the following but not limited to:

  • Registering, and admitting patients at the facility. Creating patient records. Make appointments and schedule procedures for future medical treatments.
  • Accepting incoming referrals and entering them into tracking software.
  • Managing the process of converting a referral into an active home care case.
  • Ensuring referral forms are completed correctly, and assisting with securing any supporting documentation needed.
  • Promptly advising appropriate staff of new admissions, and entering the cases into scheduling system.
  • Maintaining accurate, current records of admissions, discharges and other changes to the caseload.
  • Acting as liaison with family members and referral sources
  • Acting as a point of contact with contracted vendors.

Requirements:


  • Excellent written and verbal communication skills.
  • Knowledge of the different facets of home care.

Qualifications:

  • At least 1 year of experience in Home Care Agency.
  • Bilingual a plus but not required.
