Floral Home Care located in Queens county is looking for experienced Intake Coordinator.The Intake Coordinator is responsible for for the following but not limited to:
- Registering, and admitting patients at the facility. Creating patient records. Make appointments and schedule procedures for future medical treatments.
- Accepting incoming referrals and entering them into tracking software.
- Managing the process of converting a referral into an active home care case.
- Ensuring referral forms are completed correctly, and assisting with securing any supporting documentation needed.
- Promptly advising appropriate staff of new admissions, and entering the cases into scheduling system.
- Maintaining accurate, current records of admissions, discharges and other changes to the caseload.
- Acting as liaison with family members and referral sources
- Acting as a point of contact with contracted vendors.
Requirements:
- Excellent written and verbal communication skills.
- Knowledge of the different facets of home care.
Qualifications:
- At least 1 year of experience in Home Care Agency.
- Bilingual a plus but not required.
