Summary:


The HEDIS/GPRA Quality Coordinator (HQC) performs outbound calls to attributed patients of the clinic to close HEDIS/GPRA quality care gaps.

This includes scheduling appointments, validating documentation in the progress note, conducting surveys and questionnaires and other coordinating activities such as submitting authorizations, collecting medical records, and collaborating with various stakeholders to ensure care caps are closed.

Must be able to demonstrate knowledge and skills necessary to perform all job-related activities as outlined below. In addition, must perform other duties as assigned.


Essential Duties and Responsibilities:

Customer Service

  • Be courteous and friendly on the phone, following SDAIHC service standards and best practices.
  • Be professional at all times.
  • Provide patients with a caring and positive attitude.
  • Answer all questions without rushing the patient. Perform service recovery if patient's expectations are not met.
Patient Engagement

  • Call attributed patients to schedule medical appointments to address quality care gaps.
  • Strategically speak and engage with patients and convince hesitant patients to come in for care
  • Remove barriers to accessing care such as coordinating transportation, offering telehealth, etc.
  • Perform courtesy calls to remind patients of appointments, provide additional instructions and expectations, and initiate answering questions the patient may have.
  • Identify and assist patients in completing appropriate screenings before any visits. Screenings may be for but not limited to depression, activities of daily living, smoking status, etc.
Documentation, and Reporting

  • Accurately schedule appointments internally and document any external appointments within the EMR.
  • Accurately document all calls and activities to evaluate effectiveness and understand trends using various tools such as excel, SharePoint, Tableau, Relevant and other available tools
Care Coordination

  • Partner with medical team to close the loop on referrals related to closing care gaps to specialists such as Imaging Health Specialist, Quest, etc.
  • Schedule appointments with specialists as needed.
Collaboration

  • Team with Data Analytics department, Quality Department, Medical Department and others to access web portals, patient lists, and other data needed to successfully conduct patient calls and care coordination to close care gaps.
  • Build positive relationships with staff and leaders to engage them in Quality Management.
  • Build positive relationships with patients.
Innovation

  • Using available data and best practices, develop strategies to efficiently target various patient populations to improve HEDIS/GPRA scores.

Qualifications:

  • Bachelor's degree from an accredited college or university in Nursing, Quality, Social Work, or other health related field preferred.
  • Minimum 35 years' experience working in a healthcare quality related environment.
  • Some knowledge of clinical quality measures, best practices, medical record review, legal aspects of documentation, or medical coding and billing preferred.

Preferred:


  • Experience in Quality Coordination, Documentation Standards, Billing, and Coding knowledge.
  • Experience serving a multinational, multicultural population.
  • FQHC background.
