- Advocate for patients regarding insurance coverage, financial assistance, benefits, and resource access.
- Coordinate patient care through assessment, evaluation, planning, and implementation.
- Communicate patient needs to the care team and follow up accordingly.
- Manage continuing care plans, offer referrals, and collaborate with the CR team for referrals and aftercare.
- Ensure comprehensive clinical record maintenance through detailed documentation.
- Effectively communicate services and programs to clients.
- Complete front end setup for clients on the Electronic Medical Record (EMR).
- Conduct pre-admission assessments and confirm all necessary documentation.
- Provide regular updates to the Clinical Director on admissions, discharges, and client leads.
- Engage in client follow-ups and coordinate referrals as needed.
- Attend team meetings and provide necessary follow-ups and documentation.
- Bachelor's Degree Preferred, LADC I or LADC II Preferred.
- 1+ years of related field experience at any level of care.
- Knowledge of PHP/IOP/OP best practices required.
- Proficiency in working with Electronic Medical Records and assisting patients with benefits.
- Understanding of diverse cultures and gender-specific issues.
- Maintaining confidentiality in all interactions.
