CASE MANAGER
FULL TIME / 35 HOURS PER WEEK
Duties include:
Assist elderly consumers in developing and maintaining maximum independent living.
Responsible for assessing the needs of elderly consumers and to develop, monitor and review service plans for home care clients.
Position requires home visits to elders, coordinating service plans, monitoring client progress, corresponding paperwork, data entry and statistics.SALARY:
$46,239
