Courage to Connect Mental Health Center is an outpatient mental health practice located in Glenview, Wilmette, Lake Bluff and Glencoe.
We provide therapeutic services, psychological testing and psychiatry to the community. It is our goal to offer our patients timely, high quality mental health treatment. Celebrating our 11 year anniversary, our practice is expanding into providing psychiatric treatment.Responsibilities:
- Conduct psychiatric evaluations and medication assessments to new and existing patients
- Prescription of psychotropic medication and and followup medication.
- Provide crisis intervention as needed
- Develop a treatment plan that may include medications as well as other recommended therapies that would benefit the patient.
- Provide ongoing medication management services
- Maintain all documentation necessary for patient records
- Actively collaborate with Courage to Connect treatment team; Psychiatrist, Clinical Director, Therapist in coordinating best course of action for each patient in psychiatry department.
- Create strong relationships with patients and their families and provide education regarding medications/symptoms/illness/side effects
- Collaborate with other treatment providers as needed
- Ability to work independently, problem solve and manage multiple priorities
- Comply strictly with medical care regulations and safety standards

Qualifications:
- Currently licensed to practice as a DNP in the state of Illinois
- Preferred Psychiatric Mental Health Nurse Practitioner PMHNP in the state of Illinois
- Previous experience working with children/adolescent and adult patients in private practice a bonus, but not required.
