Long Beach, NY is seeking a strong
Staff Social Worker with long term care/nursing home experience to join their dynamic team of professionals.

Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
Provide information to the resident, and/or family member, on residents' rights and responsibilities

Qualifications:

Must have MSW or BSW
Must have experience as a Staff Social Worker
Must have long term care/nursing home experience
Must have excellent outreach, assessment, written and verbal communication skills.
Must be able to work effectively as a team.
