Job Objective:


Works under the direction of the Chief Executive Officer, the Stroke Manager directs and supervises all functions and activities of the Stroke Program, develops policies, procedures, protocols, education and community outreach to meet standards and regulations as appropriate.

Is responsible for ensuring the Stroke Program meets all Joint Commission Primary Stroke Center requirements for active certification and any other applicable standards of other regulatory bodies related to stroke.


Duties and Responsibilities:


  • Collaboration
  • Member of the Core Stroke Team and stroke multidisciplinary team. Collaborates with other core members for creation, development, and oversight of the stroke program.
  • In collaboration with the Core Stroke and multidisciplinary team, is responsible for the creation, implementation and oversight of the Stroke Program Performance Improvement Plan.
  • With collaboration with other departments within the facility, Stroke Manager is responsible for the planning, creation and maintenance of policies, procedures, pathways, order sets, and documentation tools that are in line with current clinical practice guidelines, program goals, regulatory requirements, and to support program's process improvement initiatives.
  • In collaboration with the Quality Clinical Data Coordinator oversees and participates in the collection of data for the Stroke Program to meet regulatory requirements and performance improvement needs of the program.
  • Collaborates with other departments and programs within the facility on mutually shared goals and improvement initiatives.


  • Leading and Innovating

  • Plans, designs, implements, and monitors process improvement initiatives in accordance with the hospital's performance improvement model.
  • Responsible for the development and oversight of patient education materials and processes that meet requirements of the Stroke Program and improve quality of care for patients within the program.
  • Responsible for development of staff education and competencies that meet requirements of the Stroke Program and improve quality of care for patients within the program. Works with other members of the stroke program and departments within the facility for the planning, development, implementation, and monitoring of the stroke related education and competencies.
  • Responsible for planning, preparation and coordination of Joint Commission/EMS surveys for the Stroke Program.
  • Responsible for the development and implementation and oversight of community outreach and education related to stroke to meet regulatory requirements and Stroke Program mission statement.
  • Acts as a representative of the hospital and Stroke Program at both internal and external events with other facilities, vendors and/or regulatory agencies.


  • Quality Control

  • Identifies variances in care and assists in investigation and referral to other quality monitoring programs within the facility as needed
  • Conducts concurrent review of patients within the program, reviewing plan of care for compliance with standards and clinical practice guidelines
  • Assists and participates in the coordination of patient care, performs patient assessments and nursing interventions as needed, communicates with members of the care team as needed to ensure continuum of care. Performs education and psychosocial support with patients and families as needed.
  • Completes required continuous training and education, including department specific requirements
  • Maintains High Standards of Customer Service
  • Attends Code Stroke events in the hospital and acts as a resource and support during the process to ensure timely assessments, interventions, and standards of care are met.
  • Refrains from disclosing confidential information acquired in the course of work, except when authorized and unless legally obligated to do so.
  • Refrains from using or appearing to use confidential information acquired in the course of work for unethical or illegal advantage either personally or through third parties.
  • Follows Administrative policies on Confidentiality and Privacy Practices.

Knowledge, Skills and Abilities:


Knowledge

  • Knowledge of California Nurse Practice Act, ANA Scope of Practice, ANA Code of Ethics, DMHS, Joint Commission Requirements and ability to build practice environment that supports positive results through incorporation of above with clinical practice.
  • Current knowledge of Quality principles and process improvement methodology
  • Knowledge of Regulatory and Accrediting bodies to achieve and maintain Primary Certification in Stroke and other disease specific programs as identified by the organization.
  • Knowledge of financial resource allocation, budget development, FTE allocation and compliance at unit level.
