ESSENTIAL DUTIES AND RESPONSIBILITIES:

DUTIES:

Encourage residents to participate in, and assist in completion of self-care skills and program activities.

Observe for and report any changes in residents behavior and physical and psychiatric symptoms or complaints to the Desk Nurse and/or Unit Coordinator.

Assist residents in preparing for appointments, discharge or transfer. Escort residents as assigned.
Notify residents of meals and snacks.
Prompt, lead, co-lead, groups, outings and walks.

Assist in perinea care:

assist residents who have been incontinent with changing their clothes, changing their sheets, assist with bowel and bladder retraining, provide or assist with bath or shower as needed, provide or assist with skin care for prevention of skin breakdown.

Ensure rooms are clean and prepared for new admits.
Maintain shower logs and provide support to residents to ensure they shower at a minimum of every 3 days.
Document weights, vital signs, head counts, meal log, special need rounds, and other information as assigned.
Assist with meal service.
Report to U.C./Desk nurse incidents that occur in the facility.
Assist nurses during medication pass.
Monitor smoking times.
Attend orientation and in-service as required.
Be economical and careful with facility property and supplies.
Maintain a clean and safe working environment.
Work cooperatively with other departments.
Perform other duties as assigned by the on-duty Unit Coordinator, Director of Operations, Nursing Staff, and Program Director.
Treat residents as individuals, be respectful, foster self-esteem, and independence. Call the residents by their preferred name.
Follow Title 9 Regulations and 7th Avenue Policy and Procedures.

This is a part-time position.


QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.


EDUCATION and/or EXPERIENCE:

High School graduate, or possess a General Equivalency Diploma (GED)

Be willing to gain a minimum of two (2) years of full-time experience, or its part-time equivalent, working in a mental health program serving persons with severe and persistent mental disabilities.

Such experience shall be in the direct provision of services to a programs identified residents or residents.
