Care Advisor* for consultations according to § 7a SGB XI starting immediately in full-time in the Finnentrop/Plettenberg region Your task:
As a Care Advisor* for consultations according to § 7a SGB XI, you are the local contact person. You promote the autonomy of care recipients and their relatives. Together with all parties involved, you strengthen home care.

In addition to organizational issues related to care, you also provide advice on psychosocial topics, act as a competent contact person for all questions related to care, and engage in local care networks.

You conduct your consultations through home visits, telephone calls, or video calls.


Your profile:

You are an experienced nursing professional (m/f/d), alternatively have completed training as a Social Insurance Clerk or have a degree in Social Pedagogy or Social Work with relevant professional experience.

You are empathetic and reliable in your approach and know how to put the clients and their needs at the center of your conversations.

You enjoy being out and about in your region and have a valid Class B driver's license. You are familiar with digital applications. You are looking for meaningful work with social relevance.
