Care Coordinator (m/f/d) at Pilgerhaus Weinheim and other locations in the Rhein-Neckar district

At the main house of Pilgerhaus Weinheim and other locations in the Rhein-Neckar district, our goal is to improve the daily lives of the young and adult individuals entrusted to us - through educational offerings, housing and care, counseling, support in daily life, and assistance with leisure activities.

Through our integration assistance services, we enable greater participation in social life. We achieve this through our special living arrangements, assisted living in the community, and cross-sector leisure activities. In addition, the Pilgerhaus-owned Center for Inclusion offers inclusive educational programs and health care planning for people with disabilities.

We are currently seeking to fill the position of
Care Coordinator (m/f/d) at the earliest possible date, with a workload of 50 to 100%.

Your responsibilities

Quality assurance in the area of care in connection with integration assistance
Collaboration with doctors and therapists
Supervision of nursing measures and counseling of employees
Close cooperation with the residential area managers
Coordination of quality circles for integration assistance
Coordination of night shifts

What we expect from you

Completed training as a nursing professional
Additional training in nursing management or nursing service management is desirable
Knowledge of Vivendi NG and PD is an advantage
Commitment, professional expertise, and teamwork
Interdisciplinary collaboration
