General Purpose:
Provide case management services to youth enrolled in Bridges to Employment program.
Connect youth with service according to their Individual Service Strategy (ISS) plan, in order to support informed and individualized connections to education, training, WIOA programs, DVRS supported employment, military, pre-apprenticeship and apprenticeship, and unsubsidized competitive employment.
Complete required documentation for all services as outlined by the grant.Essential Functions:
Work with Team for Outreach efforts to enroll youth eligible applicants.
Review new eligible applicants; send start letter/email within 5 business days of receiving new applicant to schedule intake.
Follow up on new applicants that do not come to their first & second appointment, collect paperwork, employable ID, and ISS Plan– not more than 15 days after receiving applicant information.
Explain and review all GW policies, procedures and handbook to new referrals, provide tours (if applicable) and overview of services.
Perform case management services required to include linking youth with other service providers or community resources such as: recreation, education, transportation, apprenticeship, in order further the youth's integration into competitive employment.
Complete required documentation (including Intakes, ISS Plans, Progress Notes, and other required documentation) within specified timeframes.Connect & schedule each youth for
intake/registration
with local county One-Stop Career Center
Obtain documentation from One-Stop Career WIOA Youth determining eligibility.
Register each youth with local county DVRS office for Vocational Rehabilitation Services.
Administer assessment to obtain interest/needs of youth and CASAS Goals (as needed) to assist in determining realistic and attainable vocational goals.
In addition to determining level of academic functioning.Score and compile all vocational test material and complete
evaluation/observation
summary reports.
Connect youth (as needed) to supportive services, benefits counseling and legal services. Follow up and document outcomes as needed.
Complete case notations on a daily basis related to consumers' behaviors, attitudes and/or characteristics observed during services.
Completes all progress notes per the progress note and paperwork compliance procedure.
Ensure all youth participate in career guidance, planning and counseling (Career Scope) by facilitating.
Facilitate groups in employability, workplace reading training, job club, resumes, cover letters, job applications, and Financial Literacy Education (Your Money Your Goals)
Facilitate groups/classes, conduct role plays, workshops on topics:
intern job search, completing job applications, discussing criminal history at interviews, expungement etc.
Prepare for sessions in advance with guidance from supervisor on planning, developing materials for each group.
Give assignments to youth, track
performance/progress
and work as a team to develop a monthly calendar for services.
Enter all youth personal and demographic information completely into Foothold Awards system.
Assist each youth with job exploration and career development (ISS Plan)
Upload all assessment data in the respective File for each youth in Foothold Awards System
Maintain good relationships and communication with local school districts, DVRS and One-Stop offices.
Organize and prioritize daily, weekly and monthly schedule. Maintain all appointments on the Outlook calendar.
Serve as an agency representative at community and social service meetings.
Collect participant attendance sheets for trainings and report monthly to Coordinator
Assist with providing resources to participants reporting barriers towards employment.
Provide on the job training for youth placed into competitive employment.
Train consumers on public transportation alternative to get to/from work.
Provide ongoing coaching to help increase retention rates and problem solve issues that may arise on the job.
Educate youth & families on working and the impact it will have on benefits that they currently receive. Connect to Benefit Counseling Services as needed for SSI/SSDI youth.
Develop 2 employment leads monthly, schedule a minimum of 5 interviews monthly.
Meet with employers, complete job task analysis at each employer sites and submit to Coordinator monthly.
Assist youth with understanding job descriptions, application process and educate employers about ADA issues, tax credits and job modifications as needed.
If fluent in another language, may be asked to translate on behalf of team as needed.Exercise good judgment and discretion in all communication and when making decisions that impact individuals, the group of individuals, and/or in crisis situations.
Develop and maintain positive & productive relationships with coworkers, supervisors, and other colleagues, individuals, parents & caretakers, support teams, and partners.
Become familiar with CARF regulations. Assure services provided and documentation maintained is CARF compliant.Attend rehab dept. and all staff meetings.

Qualifications:
Bachelor degree from an accredited school in related field required.
1 year of experience in mental health, vocational rehabilitation, or case management preferred.
Bilingual (Spanish) and/or ASL preferred.
Skills Required:
Intermediate Word, Excel and Outlook preferred
Excellent communication and written skills
Highly organized self-starter, excellent organizational and administrative skills. Must be able to prioritize and multi-task
Navigate and negotiate through difficult situations
Ability to communicate with all levels of employees
