We are currently seeking a Familial Care Worker (m/f/d) to expand our Family Care Counseling services at Albertinen Hospital/Albertinen House. This position is available immediately in full-time or part-time.

Your responsibilities will include:

  • Providing support and assistance during the transition from hospital to home
  • Counseling family caregivers on relevant home care topics such as cognitive impairments, caregiving and daily living skills, and measures to support and maintain resources and personal resilience
  • Conducting educational programs: providing information and promoting skills through practical training and workshops
  • Training and counseling on relevant family caregiving actions within the home environment
  • Advising on the design of the care network, as well as potential relief services and benefits from the nursing care insurance system
