Requirements:


  • Completed degree in Social Work
  • Indepth knowledge of social legislation, civil code, and other regulations
  • Experience in the field of healthrelated social work
  • Enjoyment of your profession, friendly and positive demeanor, and empathetic approach
  • Strong social, communication, and professional skills, as well as a high level of counseling orientation

Your tasks:


  • Optimal organization and coordination of local transfers and postclinical aftercare
  • Individual, resourceoriented, psychosocial counseling for patients and/or family members/legal guardians, and joint development of a poststationary concept
  • Counseling for patients and family members on social law issues and support with the application for social benefits
  • Referral to further counseling centers and selfhelp offers
  • Information and counseling for our patients on health, rehabilitation, intensive care, needsbased provision of aids, and services from other social security providers
  • Networking with nursing facilities, aftercare institutions, employers, as well as cost bearers and corresponding handover
