DESCRIPTION OF WORK:


  • As an Aging Care Manager 2 for the Assessment Unit, you will be assigned approximately 35 to 40 cases each month through the AAA intake and referral process. You will be responsible for conducting the initial field visit with consumers, their family members, and others, either in their homes or longterm care facilities, in order to determine an appropriate level of care. You must exercise strong critical thinking skills and the ability to accurately and efficiently prioritize work based upon assigned cases and the safety and needs of the client.

Work Schedule and Additional Information:


  • Fulltime employment
  • Work hours are 8:00 AM to 4:00 PM, Monday
  • Friday, with a 30minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY:


QUALIFICATIONS

Minimum Experience and Training Requirements:


  • Six months as an Aging Care Manager 1 or a County Caseworker 1;_ _
    or:
  • Successful completion of the County Social Casework Intern program;
    or:
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;
    or:
  • An equivalent combination of experience and training.

Other Requirements:

  • You must be able to perform essential job functions.
