Are you looking for an innovative team? Then you've come to the right place. We are currently seeking a nursing professional for our Discharge Management department (m/f/d). The position is full-time, but can also be divided.

As a member of our team, you will serve as the link between inpatient and outpatient care, ensuring a seamless transition from the hospital to home or a care facility.

Your responsibilities will include:

providing guidance and information to patients and their families as part of the discharge management process, coordinating discharge information with patients and their families for continued inpatient or outpatient care, offering individualized guidance and information to patients and their families regarding medical aids, and effectively communicating with all professional groups involved in the patient's care.
