HUP is looking for a full-time Social Work Team Leader

Responsibilities:

Support effective Social Work activities; manage performance of Social Work staff and provide them with continuing educational programs/training; collaborate with the Social Work Director to develop efficiency within the department
Address psychological, social and system issues of complex patients
Support utilization management activities: communicate with insurers, interdisciplinary care team and Physician Advisor
Lead discharge planning activities: assure timely coordination of home care, home medical equipment, placements, hospice, community resources, transportation
Participate in quality data collection and risk management referrals
The expected weekly caseload of the Social Work team leader is adjusted to ensure achievement of team leader accountabilities

Credentials:

PA License (LSW or LCSW) required

Education or Equivalent Experience:

Master's Degree in Social Work or related field required, two years of clinical experience required with five years of clinical experience preferred.
