Easterseals NH services include early childhood centers of excellence, medical rehabilitation, camping and recreation, vocational services, senior services, substance use treatment, adult day programs, community-based services, individual service options, transportation services, residential service options, and veterans services.
Job Qualifications:
- Bachelor's Degree in Human Services/Administration, OR
- Eight (8) years' related experience may be substituted for the degree
- Three (3) to five (5) years related experience
- Valid driver's license and reliable transportation
Responsibilities:
- Respond to all inquiries for Adult Programs and other programs in a prompt and appropriate manner.
- Complete initial intake interview, gathering all pertinent information and triage referrals into the program ensuring optimal program placement. This includes timely and accurate data entry of client information.
- Understand the funding sources and rates for programs. Additionally, understand the intake criteria for programs and have the ability to determine appropriate referrals.
- Be familiar with community resources and provide referrals and outreach to external agencies, as needed.
- Work with Program Supervisors to ensure optimal workflow efficiency and work with the Business Analyst to produce regular dashboard reports for leadership.
- Maintain an active wait list and regularly communicate with individuals on the wait list.
- Possess the skills to deescalate and manage difficult conversations.
- Work collaboratively with program leadership to understand program capacity and ensure a seamless transition for the customer.
- Maintain confidentiality and demonstrate appropriate discretion when representing or acting as a liaison for the supervisor with visitors and/or telephone contacts.
- Work with program staff to ensure a warm hand off and appropriate follow through.
Compensation:
$45,000 a year
Hours:
This is a full-time position, Monday - Friday, 40 hours per week
