    JOB SUMMARY:

    This position is responsible for the coordination of mobile unit - based outreach and the ongoing operation of the mobile testing unit for POWER Project, community - based outreach activities.

    The mobile unit is utilized for community - wide outreach, intended to promote HIV prevention, provide infectious disease education, administer rapid HIV testing, and offer linkage to HIV medical care at Chase Brexton; within a multitude of venues, across the Baltimore Metropolitan region, as part of the Ending the Epidemic national agenda.

    Coordination includes developing community partnerships, scheduling ongoing events and activities for the mobile unit, driving the mobile unit, and working collaboratively with the POWER Project Manager and Project Coordinator to establish and maintain a robust outreach and testing program.

    This position also serves as a Lead for the Project and maintains data collection and reporting responsibilities. Evenings and weekends are likely and are scheduled in accordance to the outreach calendar.


    MAJOR DUTIES AND RESPONSIBILITIES:
    Planning and Organizing
    • Supports the Manager in the scheduling, booking, organizing, tracking, promotion and staffing of Mobile Outreach and Saturday Clinic events.
    • Participates in the development and implementation of POWER Project's outreach plan, which aligns with CBHC strategies and grant deliverables.
    • Participates in internal and external HIV Testing/PrEP activities (special events as well as routine community activities)
    • Follows up on referrals in a timely, accurate manner.

    Checking, Examining, and Recording
    • Maintains assigned project reports, documents, and data, including but not limited to CTR reporting, Encounter data, and Dashboard documentation.

    Communication
    • Represents CBHC and the POWER Project on the local mobile unit task force and workgroups.
    • Collaborates with POWER Project and SWOD Leadership on both on-site and off-site, community events involving the mobile unit, for all CBHC locations.
    • Proactively develops and maintains positive working relationships and collaborates with community organizations in order to market CBHC's HIV and HIV prevention services, and to learn more about available community resources.

    Analytical and Critical Thinking
    • Provides HIV prevention counseling which is interactive in delivery, client centered and tailored to the individual needs of the client, whether or not HIV testing occurs.
    • Completes initial assessments and referrals for both PrEP and PEP
    • Conducts initial linkage assessments and develops action plans with assigned clients to access medical care.

    Compliance, Policy, and Procedure
    • Provides rapid HIV testing and counseling in accordance with the guidelines and regulations of the Baltimore City Health Department and the CDC.Patient Care
    • Referrals patients to on- and off-site community resources.
    • Provides clinic and triage coverage as assignedOther duties as assigned

    SKILLS AND ABILITIES:

    • Knowledge of available community resources and ability to make effective referrals and follow up
    • Decision making skills regarding priority setting and emergency situations
    • Advanced psychosocial assessment and interviewing skills
    • Supportive counseling skills and ability to work effectively with patients and families
    • Ability to work with a diverse population
    • Strong advocacy and organization skillsEDUCATION AND/

    OR EXPERIENCE:

    • Bachelor's degree in social work, public health or related field required
    • At least two years of related experience in a healthcare setting, community, social work, case management, or outreach experience required
    • Preferred: Certification in Testing, Linkage, and Care HIV testing course
    • Preferred: One or more years of specific HIV medical case management experienceWORKING CONDITIONS/
