La Mesa is currently seeking a full-time PFLEGEBERATER (M/W/D) PFLEGEBERATER*IN FÜR BERATUNGEN NACH § 7A SGB XI to join our team in Bielefeld.

As a Pflegeberater*in for consultations according to § 7a SGB XI, you will be the local contact person for individuals in need of care and their families.

Your main task will be to promote the autonomy of those in need of care and their families, and to strengthen home care together with all parties involved.

In addition to advising on organizational issues related to care, you will also provide support in psychosocial matters, serve as a competent contact person for all questions related to care, and actively participate in local care networks.

Consultations can be conducted through home visits, telephone calls, or video calls.

Your profile:

  • You are an experienced nursing professional (w/m/d), or alternatively have completed training as a social insurance clerk or a degree in social pedagogy or social work with relevant experience in the field of care. You have excellent communication skills and are able to provide empathetic and competent advice to individuals in need of care and their families. You are also familiar with local care networks and have a strong interest in promoting home care. Fluency in German is required for this position.
