Alliance Homecare is looking for a female Certified Home Health Aide to work with a client in Manhattan, NY

Client: Female, 88 years old:

Schedule: Sunday-Tuesday 8am-8pm

Client Details:

Fall risk

Ambulates with assistive devices, uses walker or four prong cane in the home. Caregiver should always offer standby assist during ambulation. Client uses wheelchair outside.
Caregiver should encourage her to go outisde and participate in old routines as much as possible.
Caregiver should offer to take her on recreational outings to museums, walks, etc.
Caregiver will be running some errands for client
Client has a shower chair in second bathroom which she uses for bathing.
Client is independent in most ADLs but caregiver will offer standby assistance as directed.

Home Health Aides will provide the following tasks as indicated on the individualized plan of care:

Perform home management tasks including housekeeping, laundry, shopping, accompanying to appointments, and errands
Prepare and serve simple modified diets according to instruction and assists with feeding
Assist with bathing, dressing and grooming
Assist with medication as specified on plan of care
Observe and report all changes to RN
Demonstrate competence in performing necessary skills
Complete all other duties assigned within the HHA scope of practice
