The Godeshöhe Rehabilitation Center in Bonn Bad Godesberg stands for multidisciplinary experience and scientific expertise in highly qualified neurorehabilitation for all levels of severity and geriatric rehabilitation.

Your responsibilities:

  • Initial consultation with patients at the beginning of their inpatient stay and analysis of their care needs
  • Creating a care plan for post-stationary care and coordinating interventions
  • Collaboration with all network partners involved in care
  • Advising patients on:
    • Long-term care insurance, disability ID, social welfare, and financial assistance
    • Outpatient care and home nursing as well as support services in the home environment
    • Short-term care, day care, full-time care, palliative care, shared living, assisted living
    • Counseling services in the patient's hometown and procurement of aids and equipment
  • Participation in regular
