V. "Supporters and Companions" sought for patients in their final or difficult phase of life.

You want to take time for patients and their relatives, providing them with professional advice and support through compassionate conversations during a time when they need guidance and assistance.

You also want to work in an organization where teamwork, openness, and trust are a given.

Then we are looking for you as a coordinator (m/f/d) with a workload of 30 hours per week to support our team and strengthen our outpatient hospice work for the entire district of Erding.

As a coordinator, you will strive to advise, support, and accompany people in their homes or in residential care facilities through home visits during palliative situations.


Your qualifications include:
a successfully completed degree in nursing or elderly care, desirable: experience in hospice/palliative care work and/or further training as a specialist in palliative care (160 hours) teamwork, high level of flexibility, and the ability to work independently and take responsibility the ability to empathize with our patients' life situations and accompany them without judgment Your tasks include, among others: coordinating outpatient hospice work through collaboration with nursing facilities, inpatient hospice, social services, doctors, psychosocial professionals, and pastoral care providing palliative and psychosocial counseling through home visits recruiting, training, and supporting volunteer hospice companions You can expect to join a small, experienced
