Weskan 'The EVIM Non-Profit Elderly Care GmbH is a large and innovative provider in the region with approximately stationary, semi-stationary, and outpatient places, as well as low-threshold offers and over 1,000 employees.

In the associated senior center Hochheim in the beautiful Hochheim am Main, 63 residents are cared for in 3 living areas.

The overarching goal is to enable a dignified, autonomous, and pain-free life through a person-centered framework.

In line with this foundation, we are looking to fill the position of a Palliative Care Officer as soon as possible.

Your future tasks:

You are responsible for and organize the content management of care and support in the accompaniment of all people in our facility in a palliative or terminal phase of life, including their relatives
You develop and implement individual care plans in collaboration with interdisciplinary teams and external partners
You provide support and guidance to staff in palliative care and ensure quality assurance and continuous improvement of care
You serve as a liaison between the facility and external hospice and palliative care services
You participate in training and education programs to continuously improve your knowledge and skills in palliative care
You promote a culture of compassion, dignity, and respect for residents and their families
You contribute to the development and implementation of palliative care policies and procedures in the facility
You maintain documentation and records related to palliative care services and ensure compliance with legal and ethical standards
You actively participate
