**Core Responsibilities:**

Provide competent nursing care safely and consistently
Respond efficiently to telephonic requests, exceeding patient expectations
Perform triage and symptom management for patients, whether present physically or not
Utilize standardized protocols for medication management
Conduct thorough medication reviews and provide patient education
Collaborate with the care team to monitor and implement care plans
Build relationships with patients and caregivers for preventative care and support
Create an engaging environment to meet the needs of patients and their families
Delegate tasks to support patient needs
Participate in care meetings to discuss patient care and operations
Deliver exceptional patient experience and perform necessary procedures
Perform point of care testing and specimen collection
Additional duties as assigned

Required Qualifications:

Active **Registered Nurse** (RN) Licensure
BLS Certification
EMR experience
Ability to maintain patient confidentiality
US work authorization
Strong communication and assessment skills

Preferred Qualifications:

Ability to collaborate with an interdisciplinary care team
Excellent computer skills
Strong problem-solving and prioritization skills
2+ years of healthcare experience as an RN
Previous clinic experience
Ability to work independently
Fluency in additional languages
