A Day in the Life at Our Facility

  • Leading, supporting, and developing the patient management team at two locations with approximately 14 employees
  • Analyzing and optimizing patient length of stay
  • Monitoring and improving the patient discharge process
  • Collaborating closely with other departments/clinics to ensure seamless flow of information
  • Actively networking with external stakeholders such as home care, rehabilitation clinics, nursing homes, authorities, and offices
  • Organizing aftercare solutions
  • Providing guidance to patients and their families
  • Ensuring compliance with legal regulations and data protection laws
  • Working according to the principles of Lean Hospital

Skills and Potential

  • Education and leadership experience in the healthcare industry and in case/care management
  • A leader who identifies with the values of warmth, expertise, and responsibility
  • Enjoying and actively participating in change processes, and working in a goal and processoriented manner
  • Proficient in using common computer systems (MS Office), knowledge of Polypoint-KIS & KISIM is an advantage
