You promote the autonomy of care recipients and their family members. Together with all parties involved, you strengthen home care.

In addition to organizational topics related to care, you also provide advice on psychosocial issues, serve as a competent contact person for all questions related to care, and engage in local care networks.

You conduct your consultations through home visits, telephone calls, or video calls.

Your profile:

You are an experienced nursing professional (m/f/d), alternatively have completed training as a social insurance specialist or have a degree in social pedagogy or social work with relevant professional experience.

You are empathetic and reliable in your approach and know how to put the clients and their needs at the center of your conversations.

You enjoy being out and about in your region and have a valid driver's license. You are familiar with digital applications. You are looking for meaningful work with social relevance.
