- Provide wound-related patient care following approved protocols
- Support in vein clinic operations after completing appropriate training
- Assist with intake, discharge, and case management tasks under supervision
- Collect patient information and prepare necessary equipment for medical staff
- Report care quality concerns to the Clinical Nurse Manager
- Collaborate with team members to ensure effective therapeutic interventions
- Perform hyperbaric oxygen therapy procedures as required
- Act as a Documentation Assistant in accordance with company policies
- Undertake other delegated duties as necessary
- High School Diploma or equivalent (GED)
- Graduate from a recognized practical nursing program
- Valid LPN/LVN license in the state of practice
- Minimum of 2 years' LPN/LVN experience, including in SNF or LTACH settings
- Prior experience in wound care is advantageous
- Proficient in Basic Life Support (BLS)
- Strong organizational and time-management skills
- Knowledge of healthcare regulatory requirements
- Excellent interpersonal and communication skills
- Customer service orientation with strong follow-up abilities
- Ability to multitask and thrive in a fast-paced environment
- Effective problem-solving skills and a calm demeanor under pressure
- Familiarity with computerized nursing systems
- Extended periods of stationary work and computer use
- Frequent keying on a computer
- Mobility, reading, writing, and communication tasks
- Handling items up to 20 pounds and with equipment assistance up to 75 pounds
- Pushing, pulling, bending, stooping, and reaching
- Color perception and good vision capabilities
- Standard office setting with occasional patient care exposure
