JOB SUMMARY:

The Rehabilitation Specialist assists the Agency by providing direct intervention and access to additional resources to clients and their families through professional assessment, service planning, evaluation, individual rehabilitation, case management and advocacy.

The Rehabilitation Specialist is self-motivated and remains current on community resources to offer assistance and education to clients and their families.

The Rehabilitation Specialist will be responsible for participation in community outreach and promote services offered by Counseling4Kids to prospects, the community, and other professionals.

The Rehabilitation Specialist works closely with the therapists and Community Partners to assure that the clients' needs/case planning are identified and met.


ESSENTIAL FUNCTIONS:


Clinical Functions:


  • Uphold the mission and core values of Counseling4Kids.
  • Provides direct services to assigned families, including but not limited to:
  • Coordinates the development of services by maintaining contact and engagement with assigned clients/families.
  • Providing support, advocacy, role modeling, parenting skills, and referral.
  • Establishes and conducts goaloriented activities for assigned clients and their families.
  • Conducts individual and family basic needs assessments.
  • Develops potential intervention efforts in collaboration with therapists.
  • Works nontraditional hours and travel to meet clients.
  • Participates in discharge planning for clients and provides summaries of service.
  • Supplies information and provides linkages.
  • Schedules and participates in meetings with internal staff and representatives from the school and community with direct interest in service outcomes. Attends parent engagement activities at schools participating in the program.
  • Engages clients by reframing problems and focusing on solutions; discussing and supporting progress towards goals.
  • During assigned inoffice days, coverage will be provided for assessments and other inoffice support items, such as completing intakes, as needed.
  • Educates the client on coping skills, anger management and positive communication skills and cultivates a sense of selfefficacy.
  • Completes documentation in Exym, for monitoring and billing purposes, of direct service with client and/or school staff, under DMH standards and guidelines.
  • Participation in community trainings.
  • Has a billing expectation of a minimum 1,200 UOS per week.
  • Develops facilitation of client and family classes.
  • Preparation of group materials and researching new methodologies for inclusion in group and individual facilitation.
  • Assures quality of service for clients by enforcing rules, regulations, and compliance requirements.
  • Measures and summarizes progress at specified intervals
  • Attends and participates in Planning Team Meetings.

Referrals and Other Administrative Duties:


  • Serves as a backup to Client Services Specialists, assisting with opening/closing/updating cases.
  • Maintains professional and technical knowledge by participating in workshops and trainings, specifically on evidencebased programming; reviewing professional publications; establishing personal linkages.
  • Assists with data collection and completing projects reports in a timely manner.
  • Contributes to team effort by consulting with other professionals; accomplishing related results as needed.

Public Relations/Outreach Duties:


  • Maintains agency credibility by establishing working relationships with sponsoring, collaborative, advisory, and related service agencies.
  • Promotes the agency by ensuring an understanding of program services available for clients; publicizing activities and accomplishments; adhering to a professional code of ethics.
  • Addresses and answers any questions proposed by our community partners and parents, regarding access to community resources.

MINIMUM QUALIFICATIONS:


  • Meets the necessary qualifications of Rehabilitation Specialists:
  • Master's Degree or two years of graduate professional education, and two years of experience in a mental health setting, as a specialist in physical restoration, social adjustment, or vocational adjustment.

-or-- Bachelor's Degree in Sociology, Psychology, or Social Work, or related field, and four years of experience in a mental health setting, as a specialist in physical restoration, social adjustment, or vocational adjustment.

  • Working knowledge of familybased service delivery, child abuse/neglect, child and adult development, and community resources available to strengthen healthy role functioning.
  • Ability to work as a team member and help clients change plan and implement change.
  • Must be able to communicate clearly; both orally and in writing.
