    I) Job Purpose


    The Day Habilitation Specialist is responsible for a variety of duties related to the overall care and education of Day Habilitation program participants.

    This includes implementation of individualized plans and behavior plans. Day Habilitation Specialist will ensure a safe and therapeutic working environment is maintained throughout the day. Staff will continually assess level of individual support necessary for learning and community experiences. Quality of services and supports are defined by program participants and are evaluated on a regular basis.


    II) Essential Functions
    A) Day Habilitation Specialist will guide and coach in a compassionate manner to increase knowledge and experiences in environment.

    B) Participate in planning conferences for assigned program participants.

    C) Comply with all oversight regulations in safety, payment, work schedules, and documentation of services.

    D) Work as part of an overall team to provided positive and educational activities daily.

    E) Input daily progress notes and information into participant program record.

    F) Share community activity ideas with Activity Coordinator.

    G) Implement activity schedule daily and maintain supplies for program activities.

    H) Document all training and progress according to unit and agency procedures.

    I) Maintain confidentially of personal information in formal and informal settings.

    J) Day Habilitation Specialist will actively participate in team meetings and share appropriate information with consideration to audience.

    K) Utilize de-escalation techniques and respectful speech with all coworkers and program participants.

    L) Keep a positive and Specialist working atmosphere for all program staff and participants.

    M) Support individuals to participate in activities that enhance connections to community and build self-esteem

    O) Performs other job duties or responsibilities as requested or assigned.

    III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and Abilities

    A) Attend and track all agency required trainings and in-services in compliance with MHMR and TX MHMR policies and procedures.

    B) Comply with all oversight regulations in safety, payment, work schedules, and documentation of services.


    IV) Internal & External Customer Service
    A) Day Habilitation Specialist is expected to work as a team member and keep a harmonious working environment with co-workers and program participants. All staff and program participants are to be treated with respect and dignity.

    B) Day Habilitation Specialist must maintain politeness and courteousness with family members, service providers, guardians and any other concerned member of the community.

    An atmosphere of friendliness and positivity is to be conveyed to all people encountered while being seen as a member of the MHMR Tarrant team.


    V) Travel
    A) Daily travel in agency vehicles is expected.

    B) Periodic travel in personal vehicle to local MHMR offices and program locations is expected.

    Minimum Requirements


    VII) Minimum Qualifications
    A)

    Minimum Education:
    High School diploma or GED

    B)

    Defined Education:
    None

    C)

    Preferences:
    a. Bilingual

    b. Six (6) months of health and human services direct care experience

    D)

    Substitutions:

    E)

    Years' Experience:
    None

    F)

    Defined Experience:
    Health and Human Services field

    G)

    License/Certifications:
    Valid Texas Driver's license

    H)

    Special Courses:
    None

    Supervisory Experience:

    VIII) Agency Requirements
    A) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.

    B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.

    C) Assigned work hours may change as the needs of the agency change.

    D) The Functional Title of this position may change as the needs of the agency change.

    E) All work will be completed within the scheduled work hours.

    All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.

    F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.

    G) Employment is at will, as well as agency needs may change.

    H) Agency dress code is to be followed at all times.

    I) Physical on-site presence, including regular attendance and punctuality, is an essential function of this position.

    Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
