    Job Summary:
    We are currently seeking a compassionate and dedicated Registered Nurse (RN) to join our Clinical Care Management team as a Day Shift Clinical Care Manager. In this role, you will be responsible for overseeing the comprehensive care of patients, collaborating with healthcare providers, and ensuring continuity of care across the healthcare continuum. The ideal candidate will have strong clinical skills, leadership abilities, and a commitment to patient-centered care.

    Responsibilities:

    • Conduct thorough assessments of patients' medical, psychosocial, and functional needs.
    • Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary healthcare team members.
    • Coordinate and facilitate care transitions, including hospital admissions, discharges, and transitions to post-acute care settings.
    • Communicate effectively with patients, families, and caregivers to provide education, support, and guidance regarding healthcare needs and treatment plans.
    • Monitor patient progress and outcomes, identifying barriers to care and implementing interventions to address patient needs.
    • Collaborate with physicians, nurse practitioners, social workers, pharmacists, and other healthcare professionals to optimize patient care outcomes.
    • Document all patient interactions, assessments, care plans, and interventions accurately and in compliance with regulatory requirements.
    • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance care delivery and patient outcomes.

    Qualifications:

    • Valid Registered Nurse (RN) license in the state of practice.
    • Minimum of two years of clinical nursing experience, with experience in care coordination, case management, or a related role preferred.
    • BLS certification required; ACLS certification may be required depending on clinical setting.
    • Strong assessment, communication, and leadership skills.
    • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
    • Commitment to providing patient-centered care and advocating for patient needs.
