    As a RN Clinical Field Staff Liaison (CFSL), the work you do every day makes a difference in the lives of our patients by ensuring quality and safe delivery of home health services.

    You'll provide support and act as a resource to clinicians in the field.

    You will assist in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management.


    Operations:

    Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns.

    Facilitate calls to and from Field staff; Monitor Home Health Aide supervisory visits.

    Patient Care:
    Work in the field with staff and patients as needed.
    Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.

    Minimize Patient Risk:
    Ensure Start of Care compliance and quality and safe delivery of home health services. Implement quality improvement programs in the field.

    Policies:

    Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.


    Teamwork:
    Assist with monthly Case Manager, Nurse and Therapy meetings. Minimum one-year home health experience is preferred
    Minimum of Three (3) years of experience as an RN in an acute care setting
    Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
