    About the Role

    LifeBridge Health is seeking a Care Management Supervisor to lead our Inpatient Social Work Team in Baltimore, MD. As a key member of our care management team, you will provide daily management and supervision of workflow activities, ensuring adherence to regulatory requirements and standards.

    Responsibilities

    • Lead and manage the Inpatient Social Work Team to achieve departmental goals and objectives
    • Develop and implement processes to improve staff compliance with regulatory requirements
    • Collaborate with multidisciplinary teams to coordinate patient care and services

    Requirements

    • Masters degree in Social Work (MSW) or related field
    • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker - Certified (LCSW-C) in Maryland
    • Certified Case Manager (CCM) certification preferred
