GENERAL STATEMENT OF DUTIES:

POSITION SUMMARY:

Under general supervision of Mental Health Supervisor (Self Determination/Transition Services), provides case management to youth and young adults age 14 to 26 who are receiving Saginaw County Community Mental Health Authority (SCCMHA) services. Utilizing a family/person centered planning process develops an individual plan of service/supports containing goals, objectives and methodologies to assist consumer to remain in the least restrictive community setting and meet treatment guidelines. Ensures plan is implemented correctly and monitors plan for effectiveness and consumer/family satisfaction. Coordinates necessary services, assesses progress, and provides written documentation to support billing. This position will be knowledgeable about and actively support culturally competent resiliency/ recovery based practices; person centered planning as a shared decision making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the resiliency/recovery process.

ESSENTIAL DUTIES AND RESPONSIBLITIES:

  1. Prepares assessments of consumer case management needs. Updates this assessment, at least annually, listing strengths and weaknesses and noting changes in consumer needs.

  2. Works with the family/consumer to plan, schedule, develop and implement a person centered plan upon admission and at least annually, thereafter.

  3. Assures that all consumers are offered the opportunity to have an independent facilitator facilitate their meeting and that all consumers are made aware of self-determination and offered the opportunity to participate in self-determination if 18 years of age or older.

  4. Develops individual service and support plan containing outcomes, support arrangements, health and safety needs and methodologies to support the consumer to remain in community settings and achieve their desired life outcomes.

  5. Works closely with Transition Age Youth specialist to ensure the focus of services reflect a process toward independence.

  6. Monitors level and quality of services provided to consumers as well as family/consumer satisfaction and documents that progress and provides feedback to consumer and caregivers concerning progress.

  7. Participates in team meetings and functions as a full team member.

  8. Links and coordinates services for consumers to assure ongoing quality of life, monitors the other professional services of the identified consumer and monitors family needs that impact the consumer.

  9. Assists consumers in securing inter-agency resources, including but not limited to individual therapy, group therapy, psychiatric services and Enhanced Health services (OT, PT, Dietician, Psychological). Participates in processes for authorization of internal and external services and supports. Actively pursues outside resources for services and supports, including use of third party coverage and public benefits. Provides information and referral for community resources.

  10. Coordinates communication of consumer status with physician's offices, the crisis unit, nurses, therapists, psychiatrists, or other professionals. Keeps residential providers, family members, and other professionals informed of changes, inter-agency resources, including individual therapy, skill building services, supported employment, substance abuse intervention, etc. Participates in processes for authorization of internal and external services and supports. Actively pursues outside resources for services and supports, including use of third party coverage and public benefits. Provides information and referral for community resources.

  11. Provides crisis intervention/support as needed.

  12. Provides data collected in a clinic or home setting to other professionals as needed. Keeps other professionals informed of status or change of consumer needs.

  13. Participates in staff meetings and in community meetings. Participates in agency or community training specific to the population served.

  14. Facilitates resolution of problems and other matters as they occur.

  15. Coordinates respite care program and in-home behavior services for youth and young adults when appropriate.

  16. May participate in positive behavior support planning and occasionally may implement behavior plan to assist youth and young adults in behavior modifications.

Prepares and maintains case management paperwork such as assessments, periodic reports, case files, progress notes, documentation, financial updates, invoices, etc.

Secures authorization for services agreed upon through the person centered planning process.

INCIDENTAL DUTIES AND RESPONSIBILITES:

  1. Performs various administrative/clerical functions such as preparing travel vouchers, making copies of documents, filing, etc.

  2. Attends meetings, seminars, workshops, and community events related to the public mental health mission.

  3. May occasionally transport consumers to and from agencies and community resources in personal automobile.

  4. Reacts productively to change and handles other essential tasks as assigned.

REPORTING RELATIONSHIPS:

Reports to: Mental Health Supervisor (Children Services)

Supervises: None

WORKING CONDITIONS/ENVIRONMENT:

Works in homes of consumers/families and provides therapeutic services to assigned consumer as a member of a clinical team. Close contact with consumers/families whom may be hostile, aggressive and potentially violent. Frequently drives automobile in all kind of weather conditions.

QUALIFICATIONS:

Education:

Bachelor's degree in a mental health related field from an accredited school. Must be able to apply and obtain a limited licensed bachelor social worker (LLBSW) within thirty (30) days upon offer of employment and obtain a licensed bachelor social worker (LBSW) within one (1) year from start date of position.

Experience:

A minimum of one (1) year supervised experience in the examination, evaluation, and treatment of youth and young adults and their families. Experience interfacing with educational system, including special education system; court system; juvenile justice system; Department of Health and Human Services system; and respite programs preferable.

Degree/Licenses:

Valid Michigan Driver's license with a good driving record.

Must be able to apply and obtain a limited licensed bachelor social worker (LLBSW) within thirty (30) days upon offer of employment and obtain a licensed bachelor social worker (LBSW) within one (1) year from start date of position.

Knowledge, Skills, and Abilities:

  1. Knowledge of mental health resources, and/or developmental disabilities, counseling, psychology.

  2. Knowledge of general child, adolescent and young adult development.

  3. Knowledge of wraparound principles and ability to work with consumer and their family to adhere to its principle.

  4. Ability to relate to all segments of the community.

  5. Analytical ability.

  6. Ability to handle individuals who may be distributive or potentially violent.

  7. Excellent communication skills.

  8. Computer literate and able to use a word processing program and at least one other software in the agency.

Physical/Mental Requirements:

  1. Hearing acuity to converse in person and on telephone.

  2. Visual acuity to observe consumers behavior, read and proofread documents and use EHR and other electronic devices.

  3. Ability to walk, stand or sit for extended periods of time.

  4. Manual dexterity to write and to operate standard office equipment (PC, Keyboard, Copy Machine, Fax Machine, etc.)

  5. Ability to lift and carry files and supplies at least 20 pounds.

  6. Strong interpersonal skills to interact with leadership, employees, consumers and the general public.

  7. Analytical skills necessary to conduct research, analyze, and interpret complex data and identify and solve problems by proposing courses of action.

  8. Ability to plan short and long range and to manage and schedule time.

  9. Ability to handle stress in meeting deadlines and dealing with large numbers of employees and/or consumers.
