- All over Nevada
- 40 hours a week for 5 days for 5 days
- 8:00 AM to 5:00 PM
- 3 days of training
- Can choose what area to cover
- Option to transition to Full Time.
- Participates in coordination of home health services, appropriately reporting the need for disciplines (CNA, OT, PT, MSW, etc.) to the case manager and/or clinical supervisor.
- Implements plan of care initiated by the case manager.
- Monitors patients' condition including fluid intake and output and compose patient charts
- Performs other duties as assigned by the RN such as but not limited to: administer injections, prescribed medications, enemas, etc.; Clean and dress wounds and assist with other basic medical care tasks; Collects samples for lab testing as assigned.
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance and adhere to the company policies and protocols.
- Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.
- PN License in the State of Nevada
- Updated Credentials
- CPR / BLS
- Negative TB Test Result
- Driver's License and Auto Insurance
- Flu Shot / Signed Declination Form
- Hep B / Signed Declination Form
- Covid-19 Vaccination / Signed Religious Exemption Form
- Immunization Records (MMR, Tdap, Varicella)
- Will be driving to patient's residences
- Must be able to lift 25 lbs
- Must be in good health and be able to fulfill the physical requirement
- Must be willing to drive all-over Nevada
- With Home Health Care experience preferred
