Job summary
The RN Case Manager is responsible to plan, organize and direct the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices.The position functions & responsibilities include complete initial start of care documentation and process,
provides a complete physical assessment and history of current and previous illness(es), regularly re-evaluates client nursing needs,
initiates the plan of care and makes necessary revisions as client status and needs change.
