    Actalent is seeking a dedicated and detail-oriented Clinical Pharmacist to join our team.

    Responsibilities:

    • Medication Evaluation: Review physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
    • Therapy Recommendations: Make recommendations for therapy changes to physicians and facility staff as appropriate.
    • Patient Database Management: Maintain patient/resident database, including current and past medical history, clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
    • Collaboration: Work closely with the healthcare team to provide clinical and technical support.
    • Communication: Document all communication with physicians, nurses, other team members, patients/residents, and their family members in the medical record.
    • Drug Policies and Procedures: Implement policies and procedures for safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
    • Regulatory Compliance: Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.
    • Error Reporting: Inform the Pharmacy Manager of medication-related errors.
    • Staff Assistance: Assist facility/agency staff personnel with safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
    • Additional Tasks: Perform other duties as assigned.

    Qualifications:

    • Pharmacist Licensure: Active pharmacist license in the state of Maine.
    • Clinical Experience: Previous experience in clinical pharmacy preferred.
    • Attention to Detail: Strong organizational and documentation skills.
    • Collaborative Approach: Ability to work effectively within a multidisciplinary healthcare team.
    • Regulatory Knowledge: Familiarity with federal and state regulations related to pharmacy practice.
