    Summary



    The Licensed Practical Nurse (LPN) at Lee County Healthcare Center is responsible for the delivery of care to Veterans in the primary care and/or specialty units. The LPN provides direct and indirect nursing care to patients and families within the clinic. Functions as a team member under the guidance of a Registered Nurse and is accountable for adhering to established policies, protocols, standards of care and standards of practice.



    Duties


    Duties include but are not limited to the following:

    • The LPN selected for primary care will work with the Patient Aligned Care Team (PACT) including a Physician, Registered Nurse, Medical Support Assistants, and other members of the ancillary team to provide care to a panel of patients.
    • The LPN selected for specialty will work in the specialty units including cardiology, dermatology, endocrinology, and EKG clinics.
    • The LPNs will work under the direct supervision of the Nurse Manager. The LPN selected must have the skills, knowledge, and determination for self-direction and concentrated effort to carry out complex practical nursing care assignments with patients ranging in age from young adulthood to the most senior years.
    The LPN must be able to:
    • Provide direct nursing care to patients in an outpatient setting.
    • Document care provided observations and responses on appropriate tools or forms, observing changes in patient's condition.
    • Take vital signs, height and weight, and records in CPRS according to policy and procedure.
    • Identify, report, and document medication errors/discrepancies, reactions and side effects immediately according to policy and procedure.
    • Provide patient education for common disease processes, medications, and treatment regimes.
    • Promote performance improvement initiatives, identifies problems, collects data, and supports change.
    • Maintain stock levels of supplies and medications.
    • Assist with treatments and procedures.
    • Document appropriately.
    • Assist in other areas as needed for patient care needs.
    • per VHA Directive 1177, you must possess current BLS certification through the American Heart Association (AHA), as you will provide direct clinical care to patients in this position.

    Work Schedule: Available tours: Monday - Friday or extended hours tour Tuesday - Saturday for a total of 40 hour work week.

    Pay: Competitive salary and regular salary increases.

    When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).

    Paid Time Off: 37-50 days of annual paid time offer per year days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

    Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

    Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
