- Provide skilled nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
- Perform comprehensive assessments of patients' health status, develop individualized care plans, and implement appropriate interventions.
- Administer medications, treatments, and procedures as ordered by the physician and ensure accurate documentation of all care provided.
- Complete OASIS documentation with a 24-hour turnaround time and maintain accurate and up-to-date patient records using Homecare Homebase.
- Collaborate with interdisciplinary team members, including physicians, therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.
- Educate patients and their families on disease management, medication administration, and home safety practices.
- Adhere to ethical and professional standards of nursing practice and maintain confidentiality of patient information at all times.
- Valid Registered Nurse (RN) license in the state of Louisiana.
- Minimum of 1-2 years of experience in home health care as an RN.
- Proficiency in Homecare Homebase as a charting system is required.
- Ability to complete 25-30 productivity units each week, with compensation for a full 40 hours of work.
- Availability for Monday-Friday schedule, with flexibility for weekend work or on-call as needed.
- Reliable transportation, valid driver's license, and active car insurance.
- Ability to pass a background check, drug screen, and provide all necessary medical documents in compliance with the Joint Commission standards.
