    Description


    The Palliative Care Advanced Practice Nurse performs the role utilizing the patient-centered coordinated care model demonstrated through the following competencies: leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making.

    Works collaboratively with other members of the practice/hospital team to assess, plan and implement care for individuals with health and safety needs.

    This position is located at Ocean University Medical Center in Brick, N.J..Hospice and palliative care experience preferred.
