    Hilltop Park Rehabilitation and Care Center -

    Summary Description:

    To provide routine daily nursing care and services that support the care delivered to patients/residents residing in the facility in accordance with the established nursing care policy and procedures and as directed by the supervisor.

    Essential Functions and Responsibilities:


    1. Work utilizing guidelines established by the facility policy and procedures.
    2. Provides care as directed by the nurse to patients/residents residing in the facility.
    3. Reports to the nursing supervisor any observations and pertinent information related to the care of the patient/resident.
    4. Documents observations related to patient/resident care, per facility standard.
    5. Provides services that support the care delivered to the patient/resident.
    6. Provides input in the formulation and evaluation of standards of care.
    7. Participates in the development of unit objectives.
    8. Participates in the identification of staff educational needs.
    9. Conducts self in a professional manner in compliance with unit and facility policies.
    10. Participates in the performance improvement process and activities.
    11. Performs other related duties as assigned or requested.
    12. Maintains patient/resident care supplies, equipment, and environment.
    13. Serves as a preceptor to new CNA staff, as delegated.
    14. Work rotating shifts, holidays, and weekends, as scheduled.
    15. Maintains knowledge of equipment set-up, maintenance, and use (i.e., restraints, monitors, drain devices, lifts, weight machines, etc.).
    16. General Patient/Resident Care

    17. Correctly identifies patient/resident prior to care.
    18. Ensures that patients/residents are approached in a kind, gentle, and friendly manner.
    19. Consistently provides respect for all patient's/resident's dignity and privacy.
    20. Maintains confidentiality of all patient/resident information.
    21. Reports all grievances and complaints made by the patient/resident to the Charge/Staff Nurse.
    22. Provides assistance with admission/transfers/discharges.

    23. Strips room after patient/resident is transferred/discharged.
    24. Moves patients/residents to accommodate unit needs.

    25. Transports specimens to the appropriate areas.
    26. Re-stock patient/resident rooms.

    27. Keeps rooms need and orderly.
    28. Removes excess supplies and linen.

    29. Reviews care plans as directed to determine if changes in daily care routine were made.
    30. Provides assistance with patient/resident care as directed but not limited to:

    31. Giving baths.
    32. Changing linens.
    33. Properly positioning patients/residents.
    34. Offering hydration.
    35. Giving AM and PM care.
    36. Taking Vital signs.

    37. Assists with restorative programs as directed:

    38. Positioning and alignment 5. Range of motion
    39. Skin integrity 6. Activities of daily living
    40. Bowel and bladder 7. Dining
    41. Ambulation 8. Emotional needs

    42. Ensures that hallways are kept clear of equipment or that it is kept on one side.
    43. Maintains the unit in an orderly fashion.
    44. Distributes and picks up meal trays.
    45. Assists in feeding patients/residents at mealtimes, as directed.
    46. Distributes, labels, and removes bedpans, urinals and emesis basins as requested.
    47. Collects routine specimens as directed.
    48. Assists with treatments as directed.
    49. Promptly answers patient/resident call lights. Provides appropriate responses to requests.
    50. Promptly removes full linen and trash barrels from unit.

    51. Maintenance of Supplies/Equipment

    52. Operates all equipment in a safe manner; only uses equipment for which training has been received.
    53. Reports defective equipment to the nursing supervisor.
    54. Ensures vacant rooms are neat in appearance.
    55. Maintains the nurse's station:

    56. Maintains supplies at par levels and arranges in an organized manner.
    57. Keeps nurse's station neat and orderly.
    58. Empties full waste containers.

    59. Accounts for equipment on rounds and notifies nursing supervisor of any missing equipment.
    60. Ensures the patient's/resident's environment is safe and clean.
    61. Keeps shower rooms neat and clean.

    62. Adherence to facility procedures

    63. Performs appropriate procedures according to establish method in the Standards of Practice or reference manual.
    64. Utilizes body substance precautions and other appropriate infection control practices with all activities.
    65. Follow safety guidelines established by the facility.
    66. Reports all accidents or incidents observed on the shift on which they occur the nursing supervisor.

    67. Documentation

    68. Document per facility policy.
    69. Accurately records vital signs on the appropriate form.
    70. Accurately records intake and output on the appropriate form.
    71. Records patient/resident weight on the appropriate form.
    72. Maintains CNA flow sheet as applicable.
    73. Document restorative program and progress towards goals.
    74. Uses only Company-approved abbreviations when recording information.

    75. Communication

    76. Reports all changes in a patients/residents condition as soon as possible to the nursing supervisor (DON,LVN,RN).
    77. Reports to nurse supervisor prior to leaving assigned area.
    78. Attends staff meetings.
    79. Reports patient/resident /employee problems to supervisor.

    80. Professional

    81. Makes decision that reflect both knowledge and good judgment.
    82. Shows evidence of awareness of own limitations and seeks assistance when necessary.
    83. Adheres to dress code and facility attendance policy.
    84. Attends committee meetings, if assigned. Reports related to the committee are given during staff meetings.
    85. Takes responsibility for own professional growth. Attends and participates in all scheduled training, educational and orientation classes, programs and activities including annual mandatory classes.
    86. Follow work schedules and completes assigned tasks according to the established standards of the facility.
    87. Notifies supervisor when tardy or absent from work within established facility time frames.
    88. Demonstrates organizational ability and time management.
    89. Respects confidentiality of patient/resident at all times (i.e., when answering telephone and/or speaking to co-workers).
    90. Cooperates with inter-departmental personnel to ensure that the needs of the patients/residents are met and maintained.

    91. Human Relations

    92. Demonstrates a positive working relationship with patients/residents, visitors and facility staff.
    93. Acknowledges authority and exhibits appropriate responses to the direction of supervisors.
    94. Spends time with patients/residents rather than other personnel.
    95. Readily assists co-workers as needed.

    Other Duties:

    13. Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.

    14. Agrees to comply with the Code of Conduct.

    Employee Standards:

    Educations: High school graduate or GED recipient preferred.

    Experience: Precious Nursing Assistant experience or student nurse preferred.

    License/Other: Completion of certification program for Nursing Assistants preferred or as required by state regulations. Must have a reliable source of transportation.

    Working Conditions:

    Works throughout the facility. Must be able to move about intermittently during working hours including standing, lifting, bending, stooping, twisting, pushing and pulling. Must be able to read, write and speak the English language. Must be able to transfer residents and assist in emergency evacuations. Interacts with residents, family members, staff, visitors, government agencies, personnel, etc. under all conditions/circumstances. Must be able to relate and work with the disabled, ill, elderly, emotionally upset and hostile people within the facility at all times. May be involved in community/civic health matters/projects. May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B viruses. Must be able to effectively communicate with the management staff, medical staff, nursing staff, and other unit supervisors. Maintains a liaison with residents, their families, support staff, etc. to ensure that the resident's needs are continually met.
