   1. Direct patient care is provided utilizing the nursing process under the direction and supervision of the RN at all times.
   2. Diagnostic test results are reviewed and abnormal results and changes in patient condition are reported to the RN, physician and appropriate healthcare team members immediately.
   3. Prescribed medications, therapies and treatments are administered in accordance with practical nursing standards at all times.
   4. Patient medical records, notes and physician orders are accurately reviewed and maintained; All patient care activities are accurately documented in accordance with documentation standards at all times.
   5. Components of the plan of care within the LPN scope of practice and competency are implemented to promote desired outcomes at all times.
   6. Assistance is provided with discharge planning and carrying out the patient's teaching plan under the direction of the RN at all times.
   7. The environment to promote patient and healthcare team safety is evaluated and maintained at all times.
   8. Compassionate basic nursing care is provided while maintaining patient privacy and confidentiality at all times.
2. Coordination of Care: This will have been satisfactorily performed when...
   1. The execution patient's plan of care is effectively collaborated with the supervising RN at all times.
   2. Physicians, RNs and health care team members are effectively assisted with data collection, assessment, planning, intervention and evaluation of patient and families care to achieve desired outcomes.
   3. As an advocate for the patient, supports the patient's psychological, ethical, spiritual and cultural needs by collaborating with the RN and healthcare team.
   4. Healthcare team members are sought out to serve as a resource to increase practice knowledge.
   5. Active participation on unit teams, work groups and/ or committees is provided as requested; anticipates needs and voluntarily assists others.
   6. Outcomes of the team/committees' work are supported and put into practice; keeps informed of Unit initiatives.

• Proficient in English, verbal and written communication skills and computer skills
