    Overview

    We are seeking a Physical Therapist to join our rehab team at HealthPro Heritage, where we strive to make a meaningful difference in the lives of our patients. As an Associate, you will have the opportunity to work diligently and provide high-quality care that promotes positive outcomes.

    This PRN position is available for a Physical Therapist who is dedicated to delivering exceptional patient care and is passionate about working in a fast-paced rehabilitation environment.

    Responsibilities

    • Evaluate resident information and develop a comprehensive plan of care.
    • Conduct thorough functional assessments and document resident progress.
    • Develop and implement individualized treatment plans with achievable goals.
    • Provide supervision and instruction to Assistants as needed.
    • Enter progress notes and maintain accurate department records.
    • Revise treatment plans as necessary to ensure optimal patient outcomes.
    • Document discharge summaries and orient residents to program services.

    Qualifications

    • Bachelor's or Master's degree in Physical Therapy.
    • Current license in the state of practice.
    • Prefer experience in a setting serving the same age/type of resident population.
    • Certification and demonstrated competency in identified areas of specialization.
    • Excellent communication and interpersonal skills.
    • Demonstrated ability to assess resident needs and develop a comprehensive plan of care.

    Key Qualities: We are looking for a Physical Therapist who is compassionate, empathetic, and committed to delivering exceptional patient care. The ideal candidate will possess strong communication and interpersonal skills, be able to work independently, and demonstrate a passion for providing high-quality care in a fast-paced rehabilitation environment.
