The Population Health Nurse Practitioner (NP) will perform a wide variety of duties in the Population Health Program.- Coordinate with other members of the Population Health team and Health management department to help develop, implement, and expand on the program
- Perform detailed HRA visits on members of the Health Maintenance Organization in an in-home or in-office setting as outlined in the Population Health Nurse Practitioner Visit Policy
