- Conduct an initial review of each patient's information and initiate contact
- Participate in daily multidisciplinary rounds (MDR) and weekly length of stay (LOS) meetings to discuss and review patient care
- Take a collaborative care approach that assesses clinical, spiritual, financial, and educational baselines to work towards realistic results
- Develop and implement discharge plans for patients to meet their needs for post-acute care services, avoid outlier days, reduce the length of stay (LOS), and reduce readmission rates
- Communicate with patients, their families, outside vendors, post-acute care providers, and teammates
- Minimum of 3 years nursing experience in an acute hospital setting.
- ADN degree
- Arizona or compact state RN license
- Prior Care Coordination experience in a clinical or insurance setting will be preferred.
- BSN
- Excellent customer service and presentation skills are a must.
- Strong interpersonal and written communication skills are essential.
- Demonstrated ability to apply analytical and problem solving skills.
- Demonstrated ability to manage multiple tasks or projects effectively. A
- Ability to work independently as needed with a high degree of detail orientation.
- Ability to work efficiently in a fast-paced environment with changing priorities.
