We are excited to invite a passionate and experienced Case Manager to join our team Homes for the Homeless (HFH) is searching for an experienced Case Manager for our Families w/ Children facility located in Staten Island, NY.Responsibilities
- Maintain and update digital and physical records of all families as required by governing agencies.
- Collaborate with clients to develop service plans based on an assessment of each family's needs.
- Assist families with connecting to community resources, support groups, and cultural services to provide social support.
