- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree
- Graduate of an Accredited Nephrology Training Program
- Board Certification in Internal Medicine
- Board Eligible or Board Certified in Nephrology
- Ability to obtain medical license in Kentucky
