    Job DescriptionDescription:

    JOB SPECIFIC FUNCTIONS:

    · Perform intake assessments and assign treatment plans per protocol.

    · Evaluate new clients as needed for medical problems or treatment needs.

    · Perform ongoing evaluation and implementation of treatment plans during initial detoxification period.

    · Communicate with the LIP regarding client needs and adjustments in the treatment plan.

    · Perform education and support activities with clients regarding medical problems or health maintenance.

    · Participate in policy/procedural development and implementation.

    · Assist in preparing medication boxes for clients.

    · Assist in medication room procedures, i.e. providing med boxes, over-the-counter medications or evaluation of medical needs.

    · Be well versed in all rules, policies, and procedures and successfully completes relevant competencies.

    · Ensure that Medical Rooms are clean and stocked.

    · Ensure that first aid supplies are available and stocked in all med rooms and first aid kits on the premises.

    · Filing of medical papers for current and past clients.

    · Schedule medical appointments after consulting with House Management and keep appointment tracker up-to-date.

    · Prepare necessary information, outside medical visit forms, papers/IDs/insurance cards prior to clients' medical appointments.

    · Request medical records from all appointments with an outside provider through appropriate personnel.

    · Performance of daily tasks and monthly audits as related to treatment room policies for such.

    · Monitoring of those who are complaining of illness- vital signs, symptoms and reporting to the LIP for treatment direction.

    · Any other tasks assigned by the Program Director, Medical Office Manager, or LIP.

    Requirements:

    QUALIFICATIONS:

    • Must have a minimum Associate Degree, Bachelor's Degree is preferable.
    • Should possess a current, valid, and unrestricted Nursing license in the State of Kentucky.
    • No minimum experience required.
    • Prefer knowledge of recovery, mental health and other co-occurring disorders and experience in working with addiction in a residential and / or intensive outpatient setting.
    • Prefer Experience in crisis intervention and conflict resolution skills.
    • Ability to work in a fast paced, dynamic, fluid and stressful environment.
    • Possess a valid Kentucky driver's license.
    • Reports to: LIP; Program Director
